A complete application package consists of five items:

 

1. Registration Form

This is the basic form required to begin the process of adoption and is used for opening a file and selecting a suitable social worker based on location.

2. Application Form

On this document, you will be asked to provide various information on your circumstances and background.  All documents and information collected remains confidential and is shared only with your social worker.  If you have any questions about how to fill it out, please call us at 1-800-935-4237.

3. Letter of Agreement

This is the more technical legal contract that explains in detail what services you can expect from both The Adoption Centre of British Columbia and your social worker and (both) your signature (s) is/are required to confirm your understanding of the terms.

4. Consent To Disclosure of Information

This is a one page document which gives us permission to request information and documents from the agency you were previously registered with or from the Ministry of Children and Family Development.  If you’ve not previously been registered with either an adoption agency or the Ministry of Children and Family Development, there is space on this form to indicate this as well.

5. Payment

Preferred payment method is cheque or money order.  Payment is broken down into two or three installments, depending on type of adoption.

For the exact amount of the initial installment, please contact our office and we will make our fee schedules available to you.  Please note that expenses may vary significantly throughout the process, depending on the type of adoption you choose and various other factors such as legal fees, mileage etc.

Application Package
To register, pease print this Application Package, fill it in by hand, and mail it to our office along with the phase one (first installment of fees) both of which are required to begin your home study assessment.  

We hope to have payment by credit card available as another option to make payment in the near future and will update here as soon as it is available.
